Karen Mandelbaum, Senior Counsel, co-presents “Medical Home Network’s Baseball Card: Home Run for Providers and Patients” during the Health Information and Management Systems Society (HIMSS) Global Conference, which runs from April 17 to 21. This session is part of the conference’s Interoperability Showcase.

Presenters will discuss the MHN Baseball Card, which summarizes high-level dynamic patient data for providers in one place. MHN incorporates the at-a-glance data integration across different provider EMRs for more than 160,000 Medicaid safety net patients. Attendees will learn how this integrated data solution supports seamless communication, enables clinical workflows, and improves care coordination.
