The Centers for Medicare & Medicaid Services (CMS) and state Medicaid programs have a keen interest in global hospital budgets, by which hospitals receive a fixed amount of revenue for the upcoming year for a specific patient population. The global hospital budget program requires shifting thinking away from a fee-for-service reimbursement model so that increased investments in primary care under the model can be offset over time by statewide savings generated by global hospital budgets.

The CMS Innovation Center is using its new All-Payer Health Equity Approaches and Development (AHEAD) model to encourage states to implement a program that would transition their hospitals from fee-for-service arrangements to a fixed budget as determined by the state’s design of its global hospital budget program. Connecticut, Hawaii, and—since the webinar was recorded—New York and Rhode Island have joined incumbent states Maryland and Vermont in participating in the AHEAD model. The CMS Innovation Center expects participant hospitals to be at the forefront of care redesign as aided by the CMS Innovation Center’s technical assistance and learning resources.
