As hospitals are increasingly being held financially responsible for the quality of care provided after a patient is discharged from the facility, many facilities are reaching out to post-acute and long-term care (LTC) facilities in order to have more influence over the care and treatment of individual patients. Bundled payment initiatives, readmission penalties, and other federal health care program payment models have made these affiliations even more critical for a hospital's financial wellbeing. Accordingly, this webinar will focus on opportunities for collaboration between health systems, academic medical centers, and post-acute care providers.
