Health reform's insurance exchanges are now less than two years away from operation. The economics of health reform depend on a broad range of people signing up for coverage and on payors and providers meeting new demands for their products and services in a cost-efficient way. Yet, previous experience shows that current health care programs for the uninsured are under-enrolled, and that the newly insured have trouble accessing health care beyond the hospital emergency department.
